INCIDENT REPORT

EENP volunteers and staff should fill out an incident report within 24 hours after

  • any incident of violence or harassment,
  • a lost dog incident (including any time a dog is found and reported to the emergency line), or
  • a medical emergency.

You can skip any fields that aren't relevant to the type of incident you experienced.

If possible, please fill out the incident report online using this link or the embedded form below. If you are not able to use the online form you can download and print a blank form and return it to EENP's office.
